GİRİŞ ve AMAÇ: Bipolar bozuklukta prognozu olumsuz etkileyen dürtüsel davranışlar ötimik dönemde de devam edebilmektedir. Anksiyete duyarlılığı dürtüsel davranışlara yönelik koruyucu bir faktör olarak görünmektedir. Çalışmamızda bipolar bozuklukta anksiyete duyarlılığı ile dürtüsel davranışlar arasındaki ilişki ve bu ilişkide etkili olabilecek klinik özelliklerin araştırılması amaçlanmıştır.
YÖNTEM ve GEREÇLER: Çalışmaya DSM-5’e göre Bipolar Bozukluk tanılı, ötimik dönemde, komorbid ruhsal hastalığı bulunmayan 70 hasta ve herhangi bir ruhsal hastalığı bulunmayan 69 gönüllü dahil edildi. Katılımcılara sosyodemografik veri formu, Hamilton Depresyon Ölçeği, Young Mani Derecelendirme Ölçeği, Algılanan Stres Ölçeği, Anksiyete Duyarlılığı İndeksi-3 ve UPPS Dürtüsel Davranış Ölçeği uygulandı.
BULGULAR: Hasta grupta algılanan stres düzeyi, Anksiyete Duyarlılığı İndeksi toplam, sosyal ve bilişsel alt boyut puanları ayrıca Dürtüsel Davranış Ölçeği sıkışıklık ve heyecan arayışı alt ölçek puanları kontrollere göre yüksek saptandı (her biri için p<0.001). Algılanan stres düzeyi, Anksiyete Duyarlılığı İndeksi toplam puanı ve üç alt boyutuyla ilişkili bulundu (her biri için p<0.001). Anksiyete duyarlılığının heyecan arayışı üzerine etkisinde algılanan stres tam aracı olarak belirlendi.
TARTIŞMA ve SONUÇ: Çalışmamızda bipolar bozukluk tanılı hastaların ötimik dönemde sağlıklı kontrollerden algılanan stres, anksiyete duyarlılığı ve dürtüsel davranışlar yönünden farklı özellikler gösterdiği saptanmıştır. Anksiyete duyarlılığı ile heyecan arayışı arasındaki ilişkide stres düzeyinin aracı etkisinin gösterilmesi; anksiyete duyarlılığı ve algılanan stres düzeyinin artmasıyla dürtüsel eylemlerin azalacağı hipotezini desteklemektedir.
INTRODUCTION: Impulsive behaviors that adversely affect prognosis in bipolar disorder may persist in the euthymic period. Anxiety sensitivity is a protective factor for impulsive behaviors. In this study, we aimed to investigate the relationship between anxiety sensitivity and impulsive behaviors in bipolar disorder and the clinical features that may be effective in this relationship.
METHODS: Seventy euthymic bipolar patients, according to the DSM-5 criteria, and 69 healthy volunteers with no comorbid mental disorders were included in the study. Sociodemographic data form, the Hamilton Depression Scale, Young Mania Rating Scale, Perceived Stress Scale, Anxiety Sensitivity Index-3, and UPPS Impulsive Behavior Scale were filled out.
RESULTS: The perceived stress level, social and cognitive sub-dimensions, and total scores of the Anxiety Sensitivity Index-3, and scores of the urgency and sensation seeking subscales of the Impulsive Behavior Scale were higher in patients than in healthy controls (p<0.001 for each). The perceived stress level was correlated with the total score of the Anxiety Sensitivity Index-3 and three sub-dimensions (p <0.001 for each). The perceived stress was a full mediator in the relationship between anxiety sensitivity and sensation seeking.
DISCUSSION AND CONCLUSION: In this study, we found that euthymic bipolar patients showed different characteristics in terms of perceived stress, anxiety sensitivity, and impulsive behaviors than healthy controls. The mediating effect of perceived stress on the relationship between anxiety sensitivity and sensation seeking supports the hypothesis that impulsive behaviors will decrease with increased anxiety sensitivity and perceived stress levels.
